51 year-old gentleman with multiple episodes of severe pain in his toes complains of marked joint swelling, erythema, warmth, and tenderness in his 1st MRP joints bilaterally and his 5th MTP joint on his left foot.  He has had a temperature of 100.6 degrees for the last few days.
