Celiac disease, non-tropical sprue

A.  Pathology

  1. Sensitivity to gluten in wheat, barley, oats & rye

  2. Flattened intestinal villi

  3. Lymphocytes/plasma cells in lamina propria

B.  Radiology

  1. Dilatation of the small bowel

  2. Hypersecretion

  3. Early on, mucosal folds are thickened but later flatten

 

Tropical sprue has a similar presentation, but has a possible relation to infection.

Remember the rule of 3:  Small bowel mucosal folds >3 mm represents fold thickening;  Proximal small bowel diameter >3 cm represents dilatation.
